Individual Account Required

In order to view the requested content and other special features on this website, you need an individual account.

  • This one-time Participant ID is needed to confirm your eligibility. You will find your Participant ID in your QHS Welcome Packet. If you don't know your Participant ID, contact QHS Support at Support@my-qhs.com or 1-888-747-7231
  • Please use date format mm/dd/yyyy.
  • You will use your chosen username each time you log-in to your QHS account. Please choose something you will remember such as your email address. Make a note of your new username as you will need it to finish this registration process, and with each future login.
    Usernames are not case sensitive and have a maximum length of 32 and may not contain spaces.
  • Passwords must be a minimum of 8 characters and are case sensitive.
  • This may be used in the future to recover your account, please enter an answer to the question above. This field has a maximum length of 32 characters, and is not case sensitive.
